Important SNF Prior Authorization (PA) Update:

October 2021 -

Due to critical capacity issues hospitals are now facing due to increased COVID rates, effective immediately: Molina is auto-approving an initial 7 day SNF stay at in-network facilities for Medicare/MMP/DSNP members in need of SNF care. No Prior Authorization (PA) is needed for the transfer and initial 7 days. SNF days beyond the initial 7 days will be subject to the usual medical records submission within 5 days and continuing stay Prior Authorization requirements. This policy will remain in effect until further notice.

Do you need to add, terminate, or make demographic changes to an existing Provider in your group?

Please notify Molina Healthcare at least 30 days in advance when you:

  • Change office location, hours, phone, fax, or email.
  • Add or close a location.
  • Add or terminate a provider.
  • Change in Tax ID and/or NPI.
  • Open or close your practice to new
    patients (PCPs only).

Complete the Provider Roster Template, which can be found under 'Forms' in the Provider Portal, and submit your changes to MHIDPNA@MolinaHealthcare.com.
