2021 Healthy Rewards and Value Added Benefits Ending Soon!

Important Information

Passport Health Plan by Molina Healthcare’s 2021 Value Added Benefits (VAB) program, including Healthy Rewards, will end on December 31, 2021. This deadline applies only to 2021 benefits. Value added benefits including Healthy Rewards for 2022 are not affected.

How to Claim Your Reward(s)

Please claim your gift card(s) and any 2021 extra benefits by January 14, 2022. The visit or screening must have occurred in 2021.

The Department for Medicaid Services (DMS) has updated its list of tornado recovery resources for our 1915(c) Home and Community Based Services (HCBS) waiver participants and providers. The listing also includes ways for individuals to help tornado survivors. 

COVID Vaccination (no phone call needed):

  1. Must be a Passport member age 5 years old or older.
  2. Received final dose of COVID-19 vaccination on or after 6/1/2021.
  3. Members that have received their booster, but not previously claimed a gift card are also eligible.
  4. Submit verification on My Molina mobile app or fax to 1-800-983-9160.
  5. Must include reward choice from the following options to avoid delays in processing:
    • Amazon
    • Wal-Mart
    • Kroger
    • CVS
  6. For all questions, please call the Passport Healthy Rewards line at 1-833-986-0072.

Asthma Management Mattress or Pillowcases:

  1. Be enrolled with Case Management.
  2. Complete the 3-month Breathe with Ease program.
  3. Your CM can order your pillowcases and enter your mattress cover size after you complete the program. 

All Other Rewards:

  1. Members can call the Passport Healthy Rewards line at 1-833-986-0072 or Member Services.
  2. Please provide the visit/screening(s) completed, the date(s) of service, and your gift card choice to the representative during your call.
